PORT FORWARDING

I've never done it before so its probably a simple solution but I am having a problem with port forwarding. I tried using simpleportforwarding but I cant get it to work. My ip website and android app work fine when I am in my house but I cannot access it from anywhere else. I have a cisco DPC3825 router. Theres a bunch of ips and I don't know what to do with them. theres a computer ip, hostname ip, router ip, and current local ip... as you can probably guess I'm not very computer savy. any help is very much appreciated.
here is what happens when I try to port forward using simpleportforwarding:

Are you sure the wifi attachment is using 192.168.0.12?
If you look in the attached devices list, it will be listed as ReefAngelWifi
